Overview
The Department of Pulmonary and Critical Care Medicine is a contemporary super specialty department established recently in Ramaiah Memorial Hospital. The department is led by two dedicated, industrious pulmonologists, who have trained extensively in the field at one of the most premiere institutes’ of the country. The department is equipped with state-of-the-art facilities at par with international standards to provide round the clock services. We deal with diseases of the airway, lungs, pulmonary vessels, pleura, thoracic cancers, complex airway interventions and sleep disordered breathing. The faculty is also trained in managing critically ill medical patients with special expertise in managing respiratory failure of various causes including severe ARDS, COVID-19 pneumonia, COPD and infections. Specialty clinics are being run for Severe Asthma, COPD, Post COVID, Interstitial Lung Diseases, Interventional Pulmonology and diagnosis of Lung Cancer.

- Interpretation of pulmonary function tests (including spirometry, DLCO, cardiopulmonary exercise testing, arterial blood gas analysis) and thoracic radiology in management of pulmonary diseases
- Biological therapies for severe refractory asthma
- Management of MDR and XDR tuberculosis
- Pulmonary rehabilitation therapy for chronic respiratory diseases
- Pulmonary assessment, risk stratification and management of perioperative pulmonary complications
- Oxygen delivery devices including domicilliary and long term oxygen therapy, CPAP and BiPAP therapies

- Diagnostic and therapeutic bronchoscopy (flexible and rigid bronchoscopy)
- Bronchoalveolar lavage (BAL), transbronchial lung biopsy (TBLB) with fluoroscopy guidance, endobronchial biopsy (EBB), endobronchial needle aspiration (EBNA) & brush cytology
- Endobrochial ultrasound guided transbronchial needle aspiration of mediastinal lymph nodes and tumours (EBUS-TBNA) including elastography
- Debulking procedures for central airway obstruction
- Placements of metallic and silicon stents for central airway obstruction, and blockers for hemoptysis
- Localization of alveolopleural and bronchopleural fistulae
- Foreign body removal, transbronchial cryo lung biopsy
- Rapid onsite cytological examination
- Radial endobronchial ultrasound and fluoroscopic guided biopsy of lung nodules
- Montgomery T tube insertion for subglottic stenosis
- Whole lung lavage
- Diagnostic and therapeutic thoracocentesis
- Tube thoracostomy and medical pleurodesis
- Pleural indwelling catheter insertion
- Medical thoracoscopy with pleural biopsy and adhesiolysis (including rigid and flexible thoracoscopy)

- Management of critically ill medical patients including non-invasive and invasive mechanical ventilation
- Management of sepsis and hospital acquired infections
- Difficult airway management: fibre optic intubation, video- laryngoscopyor bronchoscopy guided intubation, emergency cricothyroidotomy and tracheostomy
- Newer modes of ventilation including neurally adjusted ventilator assist (NAVA), adaptive support ventilation(ASV), esophageal pressure monitoring tdetermine transpulmonary pressures
- Clinical applications of Capnometry in ventilated patients
- Non invasive ventilation stratergies: CPAP, BPAP, AVAPS, ASV, NAVA
- Evaluation of difficult twean patients on ventilators
- Percutaneous dilating tracheostomy in critical care units
- Point of care ultrasonography of the lungs and the heart (POCUS), Compression ultrasonography of lower limbs, bedside 2D echocardiography and thoracic ultrasonography
- Invasive arterial monitoring and central venous catheter insertion
- Non invasive hemodynamic monitoring
- Extracorporeal membrane oxygenation (ECMO) therapy for refractory ARDS
- Sleep medicine
- Evaluation and management of sleep related breathing disorders (OSA, CSA, OHS, insomnia, sleep movement disorders, parasomnias, hypersomia, circadian sleep disorders, narcolepsy)
- Interpretation of level 1 (in hospital) polysomnography
- Continuous and bi level positive airway pressure support therapies
- Thoracic Oncology
- Tissue Diagnosis of lung and thoracic malignancies.
